Briefing documents - Looking for tips and examples

Id love to be able to give my clients a killer brief/project information document to fill out depending on the type of job and im not sure where to start.
Are there any kind coronauts out there that know of any good online resources or are able to share an example of their own?

I want to implement some better process and i think this would be a good place to start.
