How should the mask file look like for the bokeh?

Should it be BW mask where white is bokeh itself?

You can use color map to fake some crazy effects like chromatic aberrations.

I noticed that most photographers find bokeh from mirror lens to be ugly, while most CG guys think it's very attractive :]

I think it could be replicated, just need to understand the physics behind the proces.
