Hello guys,
Could you help me with achieving effect like this glass?

I guess that I have to apply gradient map in refraction map slot and add just a little bit of noise in bump slot, am I right?

Gradient in refraction glossiness slot, yes. Bump isn't needed for such material.

It depends how this material is made in real life. If it's something like steam condensing on the glass, then refraction glossiness as Romullus wrote. If it's just getting non-transparent, than opacity+diffuse color. If it's gradually turning into milky glass, then it could be even volumetric scattering.
